By submitting this form, you accept the responsibility for the temporary loan to your school, at no charge, of the 'Science Suitcases" designated above. You agree to return the suitcases and their contents by the return date in the same condition as received. You agree that the suitcases and their contents will be used only for teaching purposes and in strict accordance with the manufacturers' instructions. You further agree not to allow students to use/handle contents of the suitcases except while under your supervision or under the supervision of another responsible adult. Alabama Power Company shall not be liable to me, my school or any party for any property damage or personal injury caused by or arising out of the use of the suitcases or their contents.
